A Calcium Score of Zero Isn’t Always the All-Clear It Seems

Have you ever gotten a test result that was supposed to be reassuring — and instead raised more questions than it answered?


That happened recently with a patient in her 50s. She got her coronary calcium score back: zero. On paper, that’s the number everyone hopes for. Her doctor was ready to move forward with medication anyway, based on her other labs — and everyone, understandably, was a little thrown by the mismatch.


A CAC test is part of heart health. A coronary calcium score looks for calcified (hardened) plaque in your arteries.

The test itself is simple: you lie on a table with EKG electrodes on your chest to track your heartbeat, and a CT scanner takes pictures of your heart while you hold your breath for a few seconds. No needles, no dye — the whole thing takes about 10 to 15 minutes.


Results range from 0 to over 400. A zero score means no calcified plaque was detected, which is genuinely good information. But it's not the same as "no risk."


Here's why: arterial plaque comes in two forms, and this scan only sees one of them. Soft, non-calcified plaque is unstable and inflamed — when it tears, it can trigger a sudden blood clot with little warning. Calcified plaque is more stable and less likely to rupture, but it builds up over time and causes gradual, severe narrowing of the arteries. Because calcified plaque tends to represent a later stage of disease, a calcium score gives real insight into what's been happening in the arteries — but it can't tell you anything about soft plaque, which is often the more acute danger.


There's currently no standard test to measure how much soft plaque someone is carrying, which is exactly the gap a calcium score can't fill.


And a zero score means something different depending on who's holding it — a woman in her 50s with a zero score is working with a different risk picture than a man in his 70s with the same result.


Same “Perfect” Score, Different Full Picture

Here’s where it gets interesting. Picture her actual labs alongside that zero score:

  • Triglycerides: 148

  • HDL: 44

  • LDL: 196

  • Non-HDL: 225

  • A1C: 5.7

  • Lp(a): 24

  • ApoB: 68


On the surface, an LDL of 196 and a non-HDL of 225 look concerning enough to justify medication on their own. But her ApoB — a direct count of the actual number of artery-clogging particles in her blood — came back at 68, which is optimal.


That’s a meaningfully different story than the LDL number alone suggests.


Why This Gap Exists

LDL and non-HDL are both estimates — they infer particle number from cholesterol content, and that estimate can be off in either direction. ApoB counts the particles directly, which is why cardiology and lipid specialists increasingly treat it as the more accurate proxy for actual risk, particularly when LDL and ApoB disagree the way they did here.


Lp(a) is a separate, largely genetic marker — not something diet or exercise meaningfully changes, but useful for understanding baseline risk that a calcium score, taken alone, won’t capture. Her Lp(a) of 24 was within a lower-risk range, adding more nuance to an already complicated picture.


Why This Shows Up More in Midlife

Cardiovascular risk markers shift meaningfully around perimenopause and menopause, often faster than a single yearly panel captures. A “clean” scan or a “normal” LDL at 45 doesn’t necessarily hold the same meaning at 52. This is exactly the season where the full picture — not just the headline number — matters most.


What This Looks Like in Real Life

Scenario A: A patient gets a zero calcium score, feels fully reassured, and never asks about Lp(a) or ApoB. Medication decisions get made — or skipped — based on LDL and non-HDL alone, without the fuller context.


Scenario B: A patient with the same zero score asks her doctor to also check Lp(a) and ApoB. The results reframe the conversation entirely, and she and her doctor make a more informed decision together, rather than reacting to one number in isolation.


Same starting scan. Very different level of clarity.


Practical Shifts

1.        If you have a coronary calcium score of zero, don’t assume that closes the conversation — ask what else is on your lipid panel

2.        Ask your doctor specifically about Lp(a) (usually only needs testing once) and ApoB, especially if LDL or non-HDL run high

3.        Don’t stop or start any medication based on this post — this is a “bring the full panel to your doctor” conversation

4.        If your LDL and ApoB numbers seem to tell different stories, ask your doctor which one is driving the treatment decision, and why
